Q. What do I need to bring?

A. Here is a list of items you will need at registration:

-Social Security Number
-All active insurance cards
-Picture ID
-Any paperwork given to you by your physician
-Living Will, if applicable
-Advance Directives, if applicable
-Powers of Attorney, if applicable

All information obtained during your hospitalization will be held in strict confidence. For added personal comfort, you may also bring your own:

-pajamas or nightgown,
-bathrobe,
-slippers,
-toothbrush,
-non-electrical shaving equipment
-and other personal effects you use everyday.

We request that you leave your valuables at home. Treatment Permits We will ask you to sign appropiate consent and release forms that authorize hospital personnel to provide the services your physician has ordered for you. Identification Bands You will be required to wear an identification band during your stay. This helps ensure that you will recieve the proper treatments and medications. Please do not remove the band for any reason.

Q. Should I bring my medications from home with me?

A. Please do not bring medications from home to the hospital unless your physician instructs you to. Patients should have a complete list of the medications, with frequency and dosages available at admission. Please take any medications that you normally take in the morning, UNLESS INSTRUCTED OTHERWISE. If you normally take your medications in the evening, please take as usual. If you take Insulin, Coumadin, or Plavix you MUST speak to the anesthesiologist at the hospital 24 hours prior to your scheduled surgery.

Q. What forms of payment do you accept for services rendered?

A. You may pay by cash, check, money order, MasterCard, Visa, American Express, or Discover.

Q. What time should I report to the Hospital?

A. We ask that you report to the hospital at least 20 minutes before your scheduled procedure. This ensures that all paperwork will be properly taken care of.

Q. Where do I park?

A. Parking is available right outside of North Texas Hospital.

Q. Will I have lab work done before my procedure? If so, where do I go?

A. Your complete medical care is under the direction and supervision of your physician. Most lab work has been completed before time of surgery or treatment. Your physician will inform you otherwise.

Q. Can I drive myself to and from the hospital?

A. Please be absolutely sure you have someone to drive you home after your procedure. You WILL NOT be able to drive for 24 hours after surgery. You should have someone stay with you for 24 hours after surgery. Patients under the age of 18 MUST be accompanied by a parent or legal guardian that must remain at the hospital until the patient is discharged.

Q. May I take a bath or shower before my procedure?

A. Yes, it is fine to take a bath or shower the morning of your procedure.

Q. Under what conditions should I notify my surgeon?

A. Please notify your surgeon as soon as possible for the following reasons: If you need to cancel your surgery, for any reason. If you physical condition changes in any way (for example: you develop a fever, cold, cough, flu, chest pain, or any change in the condition for which you are having surgery).
